Finding: When FEP A01272 CHA PM need use DI water to clean. Customer take a plastic bottle that cover a DI sticker, however It contains maybe ethanol. Action: Prevent customer from using it. No one knows exactly what the contents are. suggest to scrap it. Use correct label chemicals
Finding: third party customer vendor did not barricade very well Action: remind vendor and correct them immediately, and them the safety risk.
Finding: CHJS customers are working with gas turn on due to the tools power-up, but there are no any PPE or proper barricades around the open floor, it's so dangerous and which could cause a serious fall accident. Action: 1. Remind customer to perform proper barricades with their working area;2. Inform AMAT engineers not to approach the dangerous area to ensure our own safety.
Finding: Someone took Etch tool 704184(T0 installation) barricade never return back. Action: Found spare barricade and build properly.

Finding: CHJS DDP BPSG tool need to replace the failure ozone chiller, after power off the tool by following AMAT standard procedure, customer prepare to remove the side panel and disconnect the power cable without AMAT engineer and proper LOTO program. Action: Our local team immediately informed customer to stop working, Then trained them and completed the necessary LOTO program together, and last, completed the replacement after using a multimeter to test and confirm that there is no residual electric energy.
Finding: Customer push down the TXZ heater hardly when installing the edge ring, the heater fall down immediately may cause hands hurt. Action: Tighten the heater lift coupling screws to prevent heater drop
Finding: Elevator foot level is about 5cm high than building floor, it will casue trip. Action: Feedback to customer and let customer to inform their facility to correct it.
Finding: 2022/8/10 10AM At the entrance of fab, the customer OHT vendor no set up warning signs and barricade in time when installing rails at height. It took about 10 minutes for the safety supervisor to arrive Action: Inform the customer's EHS to use barricade to isolate the appropriate work area. Avoid falling objects

Finding: OHT vendor only carry seat belts and does not hang safety belts on the shelf when high place operation Action: need to hang safety belts on the shelf
