Internal Relationship in Chinese Hospital

Participant selection and purpose of study You (i.e. the research participant) are invited to participate in a study of internal relationship in Chinese hospital. I (i.e. the investigator) hope to learn how the internal relationship affects the Chinese hospital industry. You were selected as a possible participant in this study because you are an employee in the hospital and you know how exactly the internal relationship goes on. Description of study and risks  If you decide to participate, we will ask you to complete a questionnaire which should take approximately 20 minutes. Please circle an answer for each question that seems closest to describing reality, as YOU perceive it. There are no right or wrong answers. It is your opinions and feelings that are important. We request that you respond to all the questions, as they are critical to the study.  Confidentiality and disclosure of information  Any information that is obtained in connection with this study and that can be identified with you will remain confidential and will be disclosed only with your permission, except as required by law. If you give us your permission by signing this document, we plan to publish the results. In any publication, information will be provided in such a way that you cannot be identified.  Your consent  Your decision whether or not to participate will not prejudice your future relations with the University o f Lincoln, UK. If you decide to participate, you are free to withdraw your consent and to discontinue participation at any time without prejudice. If you have any questions, please feel free to ask me.
