鼎衡船队船员述职报告Ship-crew Job Description Report
船员离船时, 按给予的链接通过手机或电脑在网页中按要求逐项填报。
述职内容主要包括：执行公司指令情况，对船员评价以及后续聘用的建议和措施，货物操作、设备仪器状况，安全记录，大油公司/CDI检查，港口国/船旗国检查，Off-hire（故障停航），航速，油耗(85%额定功率)，大事故，维护保养，坞修准备等情况。

对船舶包括人员安全、设备管用养修，货物系统操作的管理改进的建议Suggestions for Management of ship including personnel safety, equipment, cargo system maintenance and operation.
任期内船舶如有机、海损、PSC/FSC和大油检查发现的重大缺陷情况或故障停航达两天以上等情况的，请提出你个人的原因分析、改进措施的意见。If any major deficencies found in PSC/FSC/Oil Company inspection, any damage caused by equipement failure/or collision,etc. please to have your opioin on root causes analysis, corrective and preventive actions here.
对本船船舶管理团队知识、技能和安全管理的总体评价(如满意度低于6，请在第10条中说明) 。Rate your satisfaction on overall ship management team including Knowledge, skills and SQM.（If you are not satisfied, please give detail in field No. 11）
（轮机长、船长必填项）任期内船舶常态航速、主副机燃润消耗。Vsl. daily average service speed, main engine and Aux. Engine FO/LO daily consumption.
